Equipment Checkout Contract

In order to checkout equipment, I agree to the following terms and conditions:

Terms and Conditions:

  • I Agree to present two (2) forms of identification for the initial checkout of equipment, and the Weber State University Wildcard each subsequent checkout.
  • I am held responsible for any damage caused by accident, abuse, misuse, misapplication, or neglect while the equipment is in my possession.
  • I will observe the predetermined checkout limit for the equipment that I am checking out. I understand that if I fail to return the equipment in the allotted time, a fee of $0.20 per minute late will be assessed.
  • I will return the equipment in the same working condition it was in when I checked it out. The cost of replacement will be at least $2,000.00 plus any accrued overdue fines and a non-refundable $10 processing fee.
  • I understand that for any outstanding fines or damages to the equipment over $20, the University may withhold my university transcripts, awarding of degrees, certificates, etc. and permission to register until the fines or costs of damage have been paid in full (PPM 6-22-X).
  • I understand that any outstanding fine or damage to the equipment that results in a balance owing, may be turned over to the Collections Offices, which could result in additional late fees, collection fees, not to exceed 50% of the principle balance. I understand am responsible for any additional fees if turned over to a collection agency, and/or attorney fees if the account is litigated. I understand that any unpaid or delinquent balance may be reported to national credit bureaus.
  • I understand that if I fail to return the equipment within eight (8) hours of the due back time without notifying the university, it may be reported stolen and I could face criminal charges.
  • I understand that I am legally responsible for activities engaged in with the equipment I checked out, and that I will be held legally responsible for any wrongdoing or illegal activity committed using that equipment.
  • I understand that by signing this contract I am agreeing to follow the campus network acceptable use policy. Failure to adhere to this policy including the use of peer-to-peer software for file sharing will result in the revoking of my laptop checkout privileges.
  • I understand that regardless of remaining checkout time, all equipment must be returned a half an hour before closing time of the location where the equipment was checked out.
  • I understand that equipment is not transferable, meaning that equipment may only be used and checked in by the same person who checked it out. I understand that violation of this term may void my contract.
  • I understand that there may be additional terms and conditions for checking out equipment, dependent upon the specific equipment that I check out. I understand that it is my responsibility to understand these terms and conditions before checking out the equipment.
  • I understand that the duration of this contract is one academic semester, and that I will be required to sign another contract at the beginning of the next semester.

I have read and understand the above contract. I agree to abide by all the provisions in this contract. I have been advised on the use of this equipment by a Weber State University employee on duty at the time of check-out.
